Complete and return this application form to

Club Beavers - Mandeville (July 9th),
Pier 1 - Montego Bay (July 10th) or
The Quad - Kingston (July 12th)
between 12:00 noon - 8:00 pm.

Entrants must be 18 years and over to participate.

Entrants must submit a CD of audio material, containing original
mixing, no less than 10 minutes and no more than 15 minutes in
duration.

This mix must contain, at least four different genres of music :

1. Jamaican Music (Dancehall and Reggae) 2. Hip Hop/R&B
3. Soca 4. Dance/House 5. International Pop and Alternative
